A Rare Case of Frontal Lobe Abscess from Orbital Roof Blowout Fracture-Juniper publishers

Image
JUNIPER PUBLISHERS-OPEN ACCESS JOURNAL OF HEAD NECK & SPINE SURGERY Abstract Orbital roof fractures are rare. These fractures are usually associated with high velocity impact and polytrauma, and if misdiagnosed, can have serious complications. This case demonstrates that orbital roof fractures can be easily missed from plain film radiographs and that clinical history and signs are of paramount importance in assessing these cases. This case also emphasizes the importance of multidisciplinary approach in trauma care. Keywords: Frontal lobe abscess; Orbital roof fracture; Polytrauma Introduction Orbital roof fractures are usually associated with high impact trauma, accounting for approximately 1-9% of all maxillofacial fractures [1]. These fractures are mostly associated with other injuries in frontobasal trauma [2], although infants have a relatively high incidence of orbital roof fractures because of absent pneumatisation of the frontal sinus [3]. These

Pleomorphic Adenoma of the Tongue Base: A Case Report-Juniper publishers

Image
JUNIPER PUBLISHERS-OPEN ACCESS JOURNAL OF HEAD NECK & SPINE SURGERY Abstract An 86-year-old woman underwent bronchoscopy after developing aspiration pneumonia. She was found to have a tumor of the tongue base and was referred to our department. Fiberscopy revealed a pendulous mass at the tongue base. On computed tomography, a smooth pendulous mass (2cm × 1.7cm) was seen at the base of the tongue, with no deep invasion. The biopsy report indicated possible mucoepidermoid carcinoma. The risk of surgery was high due to her age and co-morbidities, so the patient and her family did not agree to resection of the tumor. Aspiration pneumonia recurred several times over several months, after which she could not take anything orally and became bedridden for weeks. To improve her quality of life by minimally invasive surgery, the tumor was excised transorally under general anesthesia. The pathological diagnosis was pleomorphic adenoma, and the surgical margins were negative

Surgical Management for Non-Traumatic Syringomyelia using Syringo-Subarachnoid Shunt “T-Tube”: An Experience of 6 Cases-Juniper publishers

Image
JUNIPER PUBLISHERS-OPEN ACCESS JOURNAL OF HEAD NECK & SPINE SURGERY Introduction The natural history of syringomyelia is highly variable. In some cases, patients do improve or stabilize without surgery, while others deteriorate even with aggressive interventions. Various surgical procedures for the treatment of syringomyelia have been introduced, including posterior fossa decompression, syringostomy, syringoperitoneal shunting. But even though satisfactory results have been reported for most of these procedures, some authors have been critical of these techniques. Through this paper, we will try to make our contribution by reporting the results we have got after surgical management of 6 patients operated for syringomyelia by syringo-subarachnoid shunt using T-Tube. Methods Patients The mean age of our patients was 55.4 years. Etiologies of our patients’ syringomyelia are various: only one patient had post traumatic syringomyelia (6 years after trauma).

Autologous Bone Marrow Derived Stem Cell (BM-MNC) Therapy for Pressure Injury in Spinal Cord Injury - A Pilot Study-Juniper publishers

Image
JUNIPER PUBLISHERS-OPEN ACCESS JOURNAL OF HEAD NECK & SPINE SURGERY Abstract Ten Spinal cord injury (SCI) cases with grade 3 & 4 pressure injury (PI) were recruited. Five cases received autologous bone marrow derived mononuclear cells (BM-MNCs) therapy and five standard wound care. Significantly reduced surface area of PI in BM-MNC treated group was observed at week 6 (p=0.04) and at week 9 (p=0.001) as compared to standard wound care group. Similarly, significant reduction of depth was observed in BM-MNC group at week 9 (p<0.05) and exudate became significantly (p=0.001) lower from 3rd to 9th week. Clearance of slough and formation of red granulation tissue was significantly higher in BM-MNC group (p=0.001). The findings of this case series indicate that the use of autologous BM-MNCs could be a feasible option for the treatment of pressure injuries in SCI cases. Keywords: Spinal cord injury; Pressure injury; Autologous bone marrow; Mononuclear cells

Bandcizer - A Tool to Assess Dosage of Elastic Band Exercises-Juniper publishers

Image
JUNIPER PUBLISHERS-OPEN ACCESS JOURNAL OF HEAD NECK & SPINE SURGERY Introduction Home based treatment or rehabilitation is one the most blooming and evolving trend among budding and established physiotherapists as it gives improvement in pain intensity, quality of life and muscle strength at the comfort of being at home. Home based exercises are often carried out by patients with help of home-based exercise equipment’s. Bandcizer is a sensor for resistance Training. It helps to assess the dosage of Thera band resistance and Intensity Figure 1. What is the use of Bandcizer? Elastic band or resistive exercise ropes are used by many now a day as it provides the sufficient resistance for doing strengthening exercises. The duration, method, repetition and technique of using such exercises are a matter of concern for the physios who recommend home based exercises. Recent studies are suggesting the fact that secondary injuries following the practice of home-based

Intra-Thyroidal Thymic Tissue May Display Radiological Features Suggestive of Malignancy - A Case Report-Juniper publishers

Image
JUNIPER PUBLISHERS-OPEN ACCESS JOURNAL OF HEAD NECK & SPINE SURGERY Abstract Introduction: Thyroid “incidentalomas” are increasing in frequency, thought mainly to be a result of the enhanced sensitivity of ultrasonography and the increasing use of this imaging technique. We present two cases of thyroid nodules in paediatric patients which were found to be intrathyroidal thymic tissue. Case report: The first case displayed ultrasonographic features that were highly suspicious for papillary thyroid cancer with microcalcification, however following diagnostic hemithyroidectomy the lesion was diagnosed as intra-thyroidal ectopic thymic tissue. Histologically, calcified Hassall’s corpuscles would account for the calcified appearance of the nodule on ultrasound scan. We describe a further case of intra-thyroidal ectopic thymic tissue in another paediatric patient who presented with a thyroid mass. Discussion: It is important to consider thymic remnant tissue

Vertebral Body Changes in the Lower Spine of Basketball Players-Juniper publishers

Image
JUNIPER PUBLISHERS-OPEN ACCESS JOURNAL OF HEAD NECK & SPINE SURGERY Abstract Professional sport activities or work may exert considerable stress on the spine. We aimed to assess the effects of loading on the deformation of the vertebral body of the twelfth thoracic vertebra (T12) and the lumbar vertebrae (L1-L5), which are subjected to considerable stress in basketball players. A total of 20 basketball players were assigned to one of two groups, based on their experience with playing basketball: under 5 years, junior group; over 5 years, senior group. All participants underwent magnetic resonance imaging; the relevant geometric variables were measured using specialized software, and the outcome measures were calculated using specific formulas; the outcome measures were compared between the junior and senior groups. The results indicated a significant difference between the two groups with respect to the compression deformity ratio at four levels (T12, L1, L2, and

Minimally Invasive Surgery for Adolescent Idiopathic Scoliosis: Anterior and Posterior Techniques-Juniper publishers

Image
JUNIPER PUBLISHERS-OPEN ACCESS JOURNAL OF HEAD NECK & SPINE SURGERY Abstract Purpose: Surgical gold standard for adolescent idiopathic scoliosis (AIS) is open posterior spinal instrumentation and fusion. This approach causes significant soft tissue disruption and paravertebral muscle detachment. This review attempts to provide an overview of the current state of knowledge of minimally invasive (MIS) surgery for AIS. Methods:  The results of MIS for adult scoliosis are briefly reviewed as the rationale for its application to AIS. A review of the two currently available MIS techniques for AIS is then given, including indications, surgical technique, and results in the literature. Results:  Thoracoscopic anterior spinal fusion and instrumentation (TASF) offers similar results and complication rates with decreased blood loss but is technically demanding and has limited indications. The posterior MIS technique seems to offer similar results and complication r
